Occupational Therapist - Homecare

$100 - 110 / per visit


The Occupational Therapist (OT) is responsible for performing client evaluations, developing and providing therapy services, and documenting services in accordance with the plan of care developed for each patient.

Essential Duties and Responsibilities:

Provides skilled occupational therapy services/interventions in accordance with physician ordersRegularly reevaluates client for rehabilitation needs, and works in collaboration with the physician and other members of the healthcare team to modify the plan of care as appropriateAssesses patient needs, plans for, evaluates and modifies care to meet goals of OccupationalTherapy InterventionsCollaborates with all disciplines to plan and evaluate team goals for each patientInstructs patient/family/caregivers on patient's total occupational therapy programAdvises, consults and instructs members of the multi-disciplinary team in specific occupational therapy modalitiesObserves and reports patient progress and/or changes in patient status to physician and appropriate multi-disciplinary team membersDirects OT treatment and develops occupational therapy plan of care; determines priority need for OTRefers individual to other disciplines as deemed appropriate and as approved by the physicianReports adverse signs and symptoms to the physicianReports client’s reaction to treatment or changes in condition to physician/Director of ClinicalServices or designeeTeaches new skills or retrains clients in once-familiar daily activities that have become difficult or unfamiliar due to illness or injuryEstablishes equipment needs to facilitate independent livingInstructs individual and/or family/caregiver in the use of various types of adaptive equipment to\\ improve function; ensures equipment is in good working orderTrains individual and/or family/caregiver in use of prosthetic and/or orthotic devicesWhen client is planning to return to work, OT performs work capacity evaluations and may refer individual to specialized vocational training in accordance with policy and regulationsProvides accurate and timely documentation according to the plan of careParticipates in care conferences or other meetings as required to ensure coordination of home health services (if applicable)Participates in quarterly medical record reviews (if applicable)Prepares and submits clinical progress-summary based on the progress of goalsProvides in-services/education programs for various disciplines as neededParticipates in peer consultation processSupervises Occupational Therapy Assistants according to company policies/procedures andState regulationsSupervises Certified Nursing Assistant (CNA)/Certified Home Health Aide (CHHA) according to company policies/procedures and State regulations (homecare setting only)Completes recertification and comprehensive assessments according to company’s policy/procedures and state regulationsParticipates in discharge planningComplies with applicable local, state, and federal laws and regulations regarding professional standards and practice standardsMay fabricate splints and instruct individual and/or family/caregiver in the use, application and benefitsPerforms other duties as assigned/necessary

Minimum Requirements:

Current licensure as an Occupational Therapist in the State of practiceMust be a graduate of an accredited school or program of Occupational TherapyMaintains certification through the American Occupational Therapy Certification Board preferredOne (1) year of prior professional Occupational Therapy experience preferred; home care and/or facility experience preferredMust possess and maintain current CPR certification in accordance with State regulation(s)Current record of PPD or chest x-ray resultsAbility to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language may be requiredComputer proficiency requiredMust be at least 18 years of age
