About the Role:
Pertemps Social Care is seeking an experienced Occupational Therapist to join Slough Council’s Discharge to Assess (D2A) team. This essential role involves supporting timely hospital discharges and applying professional clinical judgment to ensure safe and appropriate transitions of care.

Key Responsibilities:
Support and complete Trusted Assessments as requiredUse clinical expertise to screen and process hospital discharges in line with D2A protocolsWork collaboratively with health and social care professionals to ensure effective discharge planningPromote reablement and independence as part of discharge supportRequirements:
HCPC registered Occupational TherapistPrevious experience in D2A, hospital discharge, or intermediate care teamsStrong clinical reasoning and assessment skillsUnderstanding of current discharge pathways and community resources
