About the Role

We’re seeking a passionate and skilled Occupational Therapist to join our Home Health team. In this role, you’ll provide in-home evaluation and therapy for clients with physical, emotional, or cognitive limitations. You’ll help patients achieve greater independence through customized care plans, family education, and goal-oriented therapy interventions.

What You’ll Do

Conduct home-based assessments within 72 hours of referral (unless otherwise specified).

Design and implement individualized treatment plans focused on improving daily living skills and function.

Educate patients and caregivers on therapy techniques, home exercise programs, and insurance guidelines.

Participate in interdisciplinary case conferences and collaborate across care teams.

Maintain clear, accurate documentation in compliance with all home health guidelines and regulations.

Ensure care is age-appropriate across the lifespan, from infants to geriatrics.

What You’ll Need

Bachelor’s degree in Occupational Therapy required (Master’s preferred).

Current Iowa Occupational Therapy License (provisional license with supervision accepted for new grads).

Valid Iowa driver’s license, proof of insurance, and reliable transportation.

BLS certification and Mandatory Reporter certification.

0–2 years of experience in occupational therapy (home health experience preferred).

Strong communication, organization, and independent problem-solving skills.
