What you'll do The Nurse Supervisor of Dialysis is an expert Nephrology RN who will be responsible for overseeing and supervising patient care staff in the dialysis unit. They will be responsible for ensuring that patients receive high quality care, while managing the day-to-day operations of the dialysis clinic. The supervisor works collaboratively with the Director, Manager, Physicians, Social Workers, and other healthcare professionals to coordinate patient care and treatment. They must possess a broad and deep understanding of the water system as well as the CMS regulations for dialysis and maintain survey readiness in the dialysis unit at all times. On-Call may be required during non-operational hours providing direct patient care to home and hospitalized patients as needed. Home visits are required for Home Dialysis Patients. Provides management and supervision of the Hemodialysis unit Consistently provides staff supervision which focuses on professional growth and development and desired patient outcomes as monitored by staff feedback and monitoring by supervisor Interviews and makes hiring decisions for Interviewee's Consistently collaborates with the nurse manager in performing staff performance appraisals, discipline, realistic goal setting and remediation Consistently provides a staffing schedule, which meets required patient: staff ratios and takes into account staff requests as monitored by staff feedback and supervisor review Consistently supports staff education needs by scheduling in-service presentations. Facilitates staff attendance at BLS, ACLS and other educational opportunities as staffing and funds allow as monitored by staff feedback and supervisor review. Assures staff compliance to job description and healthcare requirements Consistently collaborates with patients, families and providers in creating a patient schedule which maximizes patient productivity within the framework of available staff and operating hours of the facility, as monitored by patient, family and provider feedback Regularly holds staff communication meetings, keeps minutes, follows up and facilitates solutions as monitored by staff feedback and supervisor review Participates in program development Consistently serves as an active member of the department Performance Improvement committee as monitored by supervisor Responsible for QAPI tracking and action plans to meet goals Meets with Medical Director monthly to drive QAPI goals Consistently participates in coordinator meetings, demonstrating the ability to recommend change and propose solutions through a collaborative effort with colleagues as monitored by feedback from other coordinators and supervisor review Consistently demonstrates initiative and implements approved change projects as monitored by feedback from other coordinators and supervisor review Consistently contributes to the education of colleagues as monitored by staff feedback and supervisor monitoring Responsible for home dialysis program development Consistently participates as an active member of department performance improvement committee Responsible for QAPI tracking and action plans to meet goals. Meets with the Medical Director monthly to drive QAPI goals Consistently uses current data and research in order to have the home program reflect industry trends that will improve patient outcomes as monitored by supervisor and providers Consistently develops relationships with colleagues at other home programs as monitored by supervisor Consistently works collaboratively with the interdisciplinary team, other Bassett departments and outside institutions in order to maximize the quality-of-care other disciplines provide to home patients, as monitored by colleague feedback and supervisor review Consistently works collaboratively with the Technical Services Supervisor, patients and Purchasing in order to maintain inventory control as monitored by expense reports and supervisor monitoring Provides nursing care, training, education and home support to adult ESRD patients and significant others involved in their care Trains and educates patients and significant others utilizing up to date information as monitored by patient feedback and supervisor review Consistently assesses the need for home visits and follows up appropriately as monitored by patient feedback and supervisor review (Home Dialysis Supervisor) Consistently provides support for home patients and family members while maintaining professional boundaries as monitored by supervisor (Home Dialysis Supervisor) Consistently utilizes the nursing process when assessing patients and creating individualized plan of care as monitored by supervisor Providing direct patient care as needed in response to on-call issues Collaborates with MD and staff on all hospitalized home patients providing direct patient care as needed Resource to nursing staff Provides supervision of the home dialysis program- (Home Dialysis Supervisor) Maintains Heathcare requirements, clinical staff competencies and annual validations Consistently coordinates and supervises patient training and education by colleagues as monitored by patient feedback, staff feedback and supervisor review. (Home Dialysis) Consistently coordinates and publishes monthly on call schedule for home program as well as Inpatient Dialysis Regulatory Compliance Assures 100% compliance with Crown Web Reporting Assists with data entry in to Crown Web Runs monthly reports to support QAPI activity Sustains survey readiness and all regulatory requirements Maintains up to date policies and procedures What we need from you Education: 2 Year/ Associate Degree Graduate from a Baccalaureate, Associate or Diploma school of nursing, required *** All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN’s licensed on or before 12/19/17 are/remain exempt from this requirement 4 Year/Bachelor's Degree, BSN, preferred Experience: Minimum 1 year experience in Nephrology Nursing, required Minimum 2 years experience in Nephrology Nursing, preferred Licensure/Certifications: Valid NYS Driver's License, required NYS License as a Registered Professional Nurse, required Current BLS Certification, required Current ACLS Certification, preferred CDN or CNN Certification, preferred Skills: Clinical Skills Analysis & Evaluation Listening Communication Accountability Build Relationships Employee Relations Physical Requirements: Medium Work - Exerting up to 50 lbs. for force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects Occasional balancing, crouching, kneeling, stooping Frequent grasping, lifting, seeing/monitor/computer use, standing, visual acuity (color, depth, perception) Constant typing/clerical/dexterity, hearing, pulling, pushing, reaching, repetitive motion, talking, walking, fit testing Hazards: Occasional bodily fluids/bloodborne exposure Working Conditions: Occasional noise, odors Travel: Extensive Travel - You should expect to travel every week Total Rewards Bassett Healthcare Network’s commitment to our employees includes benefit programs carefully designed with the needs and values of all our employees in mind. Pay Transparency Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards.
