This role delivers individualized care within a transdisciplinary team serving persons with brain injuries and neurological conditions. RN/LPN Position Highlights: + Hours: 6:00am-2:30pm OR 6:00am-6:30pm + Location: Ankeny, Iowa Responsibilities of the Registered Nurse (RN) and Licensed Practical Nurse (LPN): + Provide direct nursing care according to individual care plans and physician orders. + Supervise and mentor Certified Nursing Assistants (CNAs). + Coordinate care with a transdisciplinary rehabilitation team. + Support medical, dietary, and therapeutic needs of persons served. + Document and report changes in condition and participate in care planning. + Help persons served achieve their highest quality of life through skilled nursing interventions. + Support activities of daily living and encourage independence. + Ensure care aligns with facility policies and individual preferences.
