Summary of Job Function
The Advanced Practice Nurse APN specializing in Heart Failure and Transitions of Care plays a critical role in coordinating high-quality, patient-centered care across the continuum from inpatient care, discharge through post-acute and outpatient management.
This clinician serves as a key member of the cardiac team helping to reduce readmissions, optimize evidence-based therapies, and ensure smooth care transitions.
Job Qualifications/Education/Training/Certification/Licensure
MSN/DNP from an accredited nursing program requiredActive New Jersey RN and APN licenseThe ideal candidate has at least three 3 years of acute care RN work experienceRN work experience in a cardiology specialty is preferredAPN work experience strongly preferredJob-Specific Requirements of Position
Conduct comprehensive assessments of heart failure and cardiac patients during hospitalization and at points of transitionDevelop and implement individualized care plans in collaboration with cardiologists and multidisciplinary teamsCoordinate discharge planning and ensure timely follow up care, medication reconciliation, and patient educationMonitor clinical status post discharge via clinic visits, phone follow ups, or telehealth platformsFacilitate communication between acute, post acute, and ambulatory providersParticipate in quality improvement initiatives aimed at reducing readmissions and enhancing patient outcomesServe as a clinical resource for hearth failure management, diet, activity, and symptom monitoringEngage in community outreach and support transitional care clinics or initiatives as appropriateAdditional Information:
