No County Rockland Street Address Helen Hayes Hospital 51-55 Rte. 9W City West Haverstraw State NY Zip Code 10993 Duties Description The Nurse Practitioner will perform the following general duties: Performs initial medical screening on assigned patients or when consulted; Performs ongoing assessment to determine how the needs of the patient/family/caregiver will be met. Monitors treatment, progress and medications of assigned patients. Documents as per CMS regulations. Participates in weekly rounds on assigned patients; Orders medications, consultations, laboratory and radiology requests in accordance with hospital guidelines. Ensures the patient/family/caregiver is aware of medical recommendations/follow-up. Completes Discharge Summary which reflects status and plan of care post discharge. Other duties as assigned. Minimum Qualifications Reassignment: Current New York State Department of Health Helen Hayes Hospital employee with permanent or contingent permanent status as a Nurse Practitioner (Family Health).HELPS: Must possess certification and current registration as a Nurse Practitioner in a specialty practice area by the New York State Education Department.Current New York State employee with one or more years of permanent, contingent-permanent, or 55 b/c service that is eligible for transfer under Section 70.1 of the Civil Service Law to Nurse Practitioner (Family Health).Preferred Qualifications: Five years of advanced clinical experience in a hospital or SNF setting. Strong background preferred in management of chronic disease, especially cardiac and pulmonary diseases, and prevention of hospital admissions. Proven ability to work collaboratively with teams including physicians, case managers, social workers and rehabilitation staff. Discharge planning, medication reconciliation and coordination with multi-disciplinary teams is a plus. Certification in Advanced Cardiac Life Support and NYS Mandated Infection Prevention Training are requirements in this position.
