Rehabilitation We are seeking a compassionate and skilled Per Diem Advanced Practice Provider Nurse Practitioner or Physician Assistant to join our Acute Care Consult Service within the Physical Medicine Rehabilitation PMR department The APP will provide consultative services to hospitalized patients requiring rehabilitation focused care and support functional recovery across a variety of medical and surgical servicesIn this role the APP will work collaboratively with PMR physicians nursing staff and interdisciplinary teams to assess patients develop rehabilitation care plans and facilitate appropriate discharge planning and therapy recommendations The position requires strong clinical assessment skills excellent communication and the ability to function independently in a fast paced consult based environmentResponsibilities middot Perform inpatient consultations for PMR across acute care settingsmiddot Conduct comprehensive functional assessments and recommend appropriate therapy services PT OT SLPmiddot Collaborate with attending physiatrists to create and implement rehabilitation care plansmiddot Facilitate coordination of care with interdisciplinary teams including case management and therapy servicesmiddot Document findings and recommendations clearly in the medical recordmiddot Participate in quality improvement and patient safety initiatives
