Experience in at least two (2) care sectors, including Continuing Care and/or Home & Community Care. Advanced clinical & assessment skills. Competent with MS Office Suite, TEAMs, ZOOM, and tele-health knowledge. The ability to travel in inclement weather. Able to carry a 30lb medical bag and/or various equipment.
Preferred Qualifications:CNA Certification in Palliative Care. McMasters Case Management Education. Meditech and Connect Care Trained. Previous experience in leadership role. Experience in program planning, education of adults, interdisciplinary practice, and research activities.
