Opportunities to Contribute + Clinical Care: Performs history, psychosocial, and physical examinations, diagnosis of acute, chronic, and urgent/episodic primary care conditions, formulates therapeutic plans of care, ordering and interpreting laboratory and diagnostic imaging, electrocardiographic exams, medication management and provides education to patients and their families. Order therapeutic modalities such as medications, treatments, diagnostic tests, etc. Performs primary care procedures Provide primary health care maintenance of the population served, including immunizations and recommended health screenings. Counsel and instruct patients and significant others as appropriate on medications, disease, procedures, and preventive healthcare. Assess, stabilize, and determine disposition of patients with emergent conditions consistent with policy regarding emergency and consultative call services. Document clinical encounters clearly, precisely, and in a timely manner. + Care Coordination: Coordinates the care of patients to provide safe, effective, efficient, and timely patient-centered care. Liaises between clinical staff and consult services. Consults with physicians and other members of the healthcare team regarding the care of patients as appropriate. Promotes costs savings and optimal patient experience in primary care Participates in rapid cycle continuous process improvement Participates in data reporting. + Education & Leadership: Serves as a clinical resource for staff through consultation formal and informal education, review of literature and participation in seminars, conferences, and simulation. Applies, models, and teaches evidence-based treatment and care in clinical setting. Precepts advance practice and pre-licensure nursing students each semester. Participates in scholarly projects as appropriate. Minimum Qualifications + Education: Master of Science degree in nursing or higher with national board certification as a family nurse practitioner or equivalent combination of education and experience. + Experience: One year directly related experience serving as a family nurse practitioner. + Licenses/Certifications: Requires an active license as a Registered Nurse and as an Advance Practice Registered Nurse, Family Nurse Practitioner, in the state of Texas with prescriptive authority and national board certification as a family nurse practitioner. Texas Driver's license or the ability to obtain one within 30 days of employment. + Knowledge, Skills, and Abilities: Experience with electronic health record. Ability to multi-task and work cooperatively with others A well-qualified candidate will also possess + Master of Science in Nursing or Doctor of Nursing Practice preferred. + At least five years of clinical nursing experience. + Experience with computer systems required, including web-based applications and electronic health records, some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access. Experience in health professions education is preferred. + American Heart Association - Basic Life Support Certificate.
