Nurse Navigator - Pulmonary Disease
**Employment Type:** Full time **Shift:** Day Shift **Description:** **Position Summary** At Saint Francis Hospital, part of Trinity Health of New England, we are committed to providing innovative, compassionate, and mission-driven care to our community. We are seeking a dedicated Interventional Pulmonology Nurse Navigator to join our growing Thoracic Oncology and Interventional Pulmonology Program. In this specialized role, you will serve as the central guide and advocate for patients facing lung cancer, airway disorders, pleural disease, and other complex pulmonary conditions. The Nurse Navigator ensures patients and families receive timely access to advanced diagnostic and therapeutic procedures — including EBUS, robotic/navigational bronchoscopy, airway stenting, pleural interventions, and bronchoscopic lung volume reduction. By coordinating care across a multidisciplinary team of pulmonologists, thoracic surgeons, oncologists, radiologists, and primary care providers, you will streamline the patient journey, reduce barriers to care, and improve outcomes. This is a unique opportunity to be part of a highly innovative program that is reshaping the way pulmonary care is delivered in the Hartford region. **Key Responsibilities** Patient Navigation & Advocacy + Serve as the primary point of contact for patients referred to the Interventional Pulmonology (IP) program. + Provide education, support, and guidance to patients and families before, during, and after procedures. + Identify barriers to care and connect patients with community and hospital resources. + Care Coordination & Clinical Support + Coordinate procedures, diagnostic testing, consultations, and follow-up appointments. + Communicate test results and care plans with patients, families, and referring providers. + Triage and respond to patient calls, escalating clinical concerns when needed. + Multidisciplinary Team Collaboration + Actively participate in weekly tumor boards and case conferences. + Ensure all relevant clinical information is available for team-based decision making. + Facilitate communication among providers to deliver seamless, patient-centered care. Program Growth & Quality Initiatives + Support identification of patients eligible for advanced procedures (including lung cancer screening and BLVR candidates). + Collect and track program metrics, outcomes, and patient satisfaction data. + Participate in quality improvement initiatives and community outreach related to lung health. + Professional Standards + Maintain current clinical knowledge in interventional pulmonology, thoracic oncology, and advanced pulmonary procedures. + Serve as a mentor and resource for nursing staff, trainees, and colleagues. + Uphold Trinity Health of New England’s mission and values in every patient interaction. Qualifications + Registered Nurse (RN) with current Connecticut state licensure required; BSN preferred. + Minimum 3 years of nursing experience; pulmonary, thoracic, oncology, or procedural nursing strongly preferred. + Prior experience in care navigation, case management, or coordination preferred. + Strong organizational, communication, and critical thinking skills. + Demonstrated ability to work independently and as part of a multidisciplinary team.
