​Position Summary:

The Nurse Navigator is a key member of the multidisciplinary team functioning as an advocate, educator and facilitator of care. Responsible for ensuring all referred patients receive quality and comprehensive services throughout the continuum. Coordinates the overall subspecialty programmatic goals and outcome. This role works in a fast-paced environment within a growing organization

What the Orthopedic Nurse Navigator will need:

Bachelor’s degree in Nursing preferred.Current State of Michigan Registered Nursing License, Certified Nurse in subspecialty (OCN, CNRN) required. Additional subspecialty certifications preferred.A minimum of three (3) years experience in an acute care setting.Case management experience or patient care leadership experience preferred. Five years experience in subspecialty is preferred.

What the Orthopedic Nurse Navigator will do:

Completes a comprehensive health assessment of the patient’s physical, psychological, social, environmental, financial and functional status in collaboration with multidisciplinary team members. Includes assessment of past and present treatment course.Assists in organizing and coordinating timely patient appointments.Able to understand and develop goals, treatment plan and provider options in concordance with evidenced based standards and clinical trials.Works directly with physician lead to review diagnostic studies, clinical history and course of treatment to identify most appropriate next course of care.Collaborates with multidisciplinary team members in performance improvement activities including assessment, planning, implementation and evaluation of clinical outcomes and processes for patients.
