Nurse Navigator Oncology (FT) - Seacoast

Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.  

Maintains a professional image and exhibits excellent customer relations to patients, visitors, physicians, and co-workers in accordance with our Service Excellence Standards and Core Values.Functions as a part of the multidisciplinary teamServes as resource for answering questions related to Breast Health  (as applicable)Serves as community outreach liaison, providing education about cancer prevention, early detection and screening programsCoordinates care with patients who are at high risk for breast cancer. Supports programmatic development through maintaining strong, professional relationships with clinical care Tracks and manages follow up for patients with identified needs as applicableServes as liaison between patients and clinical care providers, facilitating communication and coordination of care, facilitation of appointments, or other navigational services as needed.As requested, provides reports to administrationIs knowledgeable of and adheres to American College of Surgeons accreditation standards.Represents the organization in a positive and professional manner at all community events. Completes annual training and annual educational requirements on time.

Qualifications/Training:

3 years of relevant clinical experienceStrong communication skills – verbal and writtenAbility to work independentlyComputer literacy in Microsoft applications

 

Licenses/Certifications/Registrations/Education:

Must possess a valid RN license from South Carolina or other compact stateOncology Certified Nurse (OCN), Certified Breast Care Nurse (CBCN), Oncology Nurse Navigator-Certified Generalist (ONN-CG) or other specialty certification within 1 year of hire preferred.
