Nurse Navigator
Position OverviewUnited Health Services (UHS) is seeking a compassionate and detail-oriented Nurse Navigator to support our Medical Oncology practice. In this essential role, you’ll guide patients and their families through the complexities of cancer care—coordinating appointments, explaining treatment plans, and ensuring seamless communication between providers and support services.

Compensation Range:

$37.00 - $55.50 per hour, depending on experience

Job Responsibilities:

Identify and manage a caseload of patients based on the presence of designated condition(s), ensuring timely and appropriate navigation services.

Facilitate seamless transitions of care across the healthcare continuum, both within and outside of the UHS network, coordinating services to minimize fragmentation.

Recognize the trajectory of each patient’s disease process and implement patient-centered interventions tailored to their current and anticipated needs.

Establish individualized care goals and develop a comprehensive navigation plan in collaboration with the patient, family, and care team.

Educate patients and families on condition-specific changes, including recognition of red flag symptoms and when to seek medical attention.

Assess learning styles and health literacy to deliver effective, understandable patient and family education throughout the care experience.

Identify and address barriers to care by connecting patients with appropriate resources, adjusting care goals, and coordinating with additional team members as needed.

Promote patient and family engagement in healthcare decision-making, encouraging active participation during and beyond the navigation period.

Empower patients and caregivers with tools and knowledge to support ongoing self-management of their condition(s), promoting independence and confidence.

Collaborate with Palliative Care, Hospice, and other support services when appropriate to ensure comprehensive, compassionate care.

Document all navigation activities, assessments, and interventions accurately and thoroughly in the electronic medical record.

Serve as a patient advocate, respecting individual values, preferences, and circumstances, while participating in UHS and community initiatives that support patient outcomes and program development.

Position Qualifications:

Minimum Required:

Bachelor’s degree in nursing and

5 years health care experience

NYS Registered Nurse license

Basic Life Support (BLS) certification (or obtained within 6 months of employment)

Valid NYS driver’s license

Preferred:

Condition-specific experience in either/both inpatient or outpatient setting(s).

PA Registered Nurse license

Nurse Navigation Certification
