Job Description
We are seeking a dedicated and experienced Nurse Manager to lead and support our clinical teams. The ideal candidate will oversee patient care, ensure compliance with regulatory standards, and manage documentation processes. This role requires a strong leader with excellent communication skills and a commitment to quality assurance.
ResponsibilitiesSupervise and mentor field nurses, providing education and conducting performance evaluations.Oversee admissions, care planning, and case management; monitor patient progress and adjust plans as needed.Ensure adherence to CMS guidelines, state regulations, and agency policies through chart reviews and home visits.Ensure timely and accurate completion of OASIS, Plans of Care, and 485s documentation.Coordinate with physicians, caregivers, and families to develop individualized care plans.Assist with hiring, onboarding, and ongoing staff training.Participate in QAPI, audits, surveys, and policy development.Utilize EMR (Homecare HomeConnect) and Microsoft Office for documentation and reporting.Provide clinical support during on-call shifts as needed.Essential SkillsActive RN License; BSN preferred.5–7 years of nursing experience, with 3–5 in home health.1–2 years of supervisory/management experience.Expertise in OASIS, care planning, and CMS guidelines.Proficient in EMR systems (Homecare HomeConnect) and Microsoft Office.Excellent communication, critical thinking, and problem-solving skills.Valid driver’s license and ability to travel locally.Additional Skills & QualificationsAbility to mentor, motivate, and develop clinical teams.Deep understanding of CMS guidelines, state survey requirements, and compliance standards.Experience with QAPI programs and implementing corrective action plans.Strong interpersonal skills for working with physicians, caregivers, and families.Ability to manage multiple priorities, deadlines, and on-call responsibilities.Comfortable with policy changes, new technologies, and evolving industry regulations.Work Environment
Work primarily takes place in the office from 8:30 AM to 5:00 PM, with occasional field visits within the surrounding towns of Greenwich in Fairfield County as needed. Pay and Benefits
The pay range for this position is $140000.00 - $150000.00/yr.
Paid Time Off & Holidays
15 PTO days annually after 90 days, usable for vacation or sick time.
Accrual: +1 day per year of service, up to 20 days max.
8 paid holidays: New Year’s Day, MLK Day, Memorial Day, Juneteenth, July 4th, Labor Day, Thanksgiving, Christmas.
Flexibility: Unused PTO beyond 15 days can be paid out upon request.
