Nurse-LPN Job Details Job Location Gloversville-NPCS - Gloversville, NY Position Type Full Time Salary Range $27.00 - $28.00 Hourly Travel Percentage Up to 50% Description As a Licensed Practical Nurse in Foster Care, you will coordinate appropriate medical care, dental care, and mental health services nursing care/services to a youth and adolescent population with emotional, behavioral and physical needs. You will ensure that medical treatment services meet the requirements of the regulating agency. You will report to a Registered Nurse and you will work onsite. Responsibilities: + Communicate with medical providers to ensure proper medical care in accordance with state regulations. + Provide client and family education regarding medications and treatments. + Work within the multidisciplinary team to ensure proper communication for medial needs + Travel to foster homes to review and collect medication logs. + Nursing documentation using EHR. + Update discharge planning needs. + See foster children in foster homes as needed. + Provide information to be included on the medical portion of treatment plans for children in care. + Other duties as assigned within scope of practice and based on agency needs.
