Job Summary:

In accordance with the Mission, Vision, Values and strategic direction of PHSA, safety, including both patient and employee safety, is a priority and a responsibility shared by everyone at PHSA. As such, the requirement to continuously improve quality and safety is inherent in all aspects of this position.

Reporting to the Program Manager, the Nurse Clinician, PIVOT Program, provides and coordinates nursing practice activities related to the care of children and youth requiring antimicrobial therapy, inotrope therapy, hydration therapy, and/or palliative care while living in their home communities. This includes patient intake and referral triaging, initiating, facilitating and coordinating pediatric intravenous (IV) outpatient therapy (PIVOT); working in collaboration and liaising with families, health professionals and regional/community professionals and agencies; coordinating and facilitating teaching and support for children/youth and their families; providing educational sessions and material to staff and community health care professionals and agencies on PIVOT and vascular access in a manner that is consistent with established policies, standards and philosophies at BC Children’s Hospital (BCCH).

Duties/Accountabilities:

1.    Provides clinical leadership to the interdisciplinary team by methods such as coordinating patient care and quality assurance activities, providing information and consultation to nurses, other health care providers and students regarding PIVOT and vascular access, and ensuring compliance with standards of practice.

2.    Provides work direction to staff by methods such as reviewing workload and scheduling, coordinating and adjusting work activities and the use of staff and equipment in collaboration with the VAT leadership team.

3.    Participates in the performance appraisal process for staff by evaluating the work of others and providing written and/or verbal feedback to the Program Manager, Clinical Nurse Coordinator, and/or Clinical Nurse Educator.

4.    Initiates and coordinates patient assessments to determine the qualification for BCCH PIVOT program, responds to enquiries regarding the program and eligibility, provides anticipatory guidance and case management, participates in and oversees the coordination of care in community throughout the child/youth’s health journey with the program.  

5.    Coordinates the PIVOT Program such as liaising with regional based healthcare professionals and other programs/departments, planning clinic schedules and overseeing clinic bookings, ensuring the maintenance of PIVOT databases and patient records, streamlining clinic processes, and monitoring and reporting expenditures.

6.    Responsible for the assessment and development of individualized health care plans to support the children/youth in their community setting by performing duties such as receiving, collecting and documenting child/youth specific information related to the their illness, disease, treatment and/or family history, collaborating and problem-solving with families and other health care team and community professionals/agencies, assessing the appropriateness of service needs, and triaging as necessary to mitigate delays. Facilitates re-direction/referral of children/youth/families to other community resources, such as communities with outpatient pediatric programs as applicable.

7.    In collaboration with the Clinical Nurse Educator, identifies the educational needs of the staff, and develops, and implements training/orientation programs and ongoing staff development, including producing educational materials as required. Teaches and acts as a clinical resource for nursing staff by sharing clinical expertise, providing clinical guidance, and demonstrating and explaining the clinical skills required for care, maintenance, and troubleshooting of central venous catheter (CVC) lines, pumps and other equipment and parts and introducing new skills/procedures.

8.    Provides patient education and training by establishing therapeutic relationships with patients and families; assessing patient/family learning needs, readiness and ability to learn; providing information, explaining and involving patient/family in the treatment process; and  teaching and supporting patient requiring PIVOT services such as CVC and infusion device care, maintenance, and troubleshooting. Advocates for and facilitates informed choices.

9.    Provides clinical support and ongoing care to patients/families regarding OPAT by methods such as conducting ongoing clinic and/or virtual care assessments; providing information on program and community health resources; determining continued eligibility in the PIVOT program and level of services required; and establishing the need for patient assessment at BCCH and/or referring patients to a health care facility in their community. Performs procedures to address line and pump repairs, blockages, dressing changes, and other complications.

10.    Acts as a liaison between the program and the interdisciplinary team and community health and educational services and agencies as appropriate to clarify information, responsibilities and recommendations to ensure the needs of the child/youth and family are addressed. Liaises with in-house BCCH pharmacy and community pharmacy regarding medications and the required supplies for home deliveries based on patient needs.

11.    Assists the PIVOT Medical Director, Program Manager, and Clinical Nurse Coordinator with planning, developing, and evaluating policies, procedures and operational requirements of the program by observing service needs and providing input on addressing patient and family needs, program operations and promotes effective use of resources. Collaborates and assists the department heads and physicians with the development and maintenance of the referral checklist for physicians to refer patients into the PIVOT program.

12.    Provides educational sessions (in-person and/or virtual) on PIVOT and vascular access to other nursing staff and health care professionals at health care facilities and community settings across BC and Yukon. Produces written material and online resources for the public and provides presentations to community organizations as required.

13.    Participates in quality management activities and working groups and committees for the program and nursing practice through data collection, assessment, planning, implementation, and evaluation of strategic interventions to improve practice.

14.    Participates in approved nursing and interdisciplinary research and special projects through methods such as identifying research topics/issues, promoting staff awareness and involvement in research activities, supporting evidence-based nursing practice and gathering and summarizing data to improve education and clinical effectiveness.

15.    Performs other related duties as assigned.

Qualifications:

Education, Training and Experience

•    Current practicing registration as a Registered Nurse with the British Columbia College of Nurses & Midwives (BCCNM).
•    Eligible for registration as a Registered Nurse with the Yukon Registered Nurses Association (YRNA)
•    Completion of post basic specialty nursing certificate/program if applicable, and three (3) years recent related experience in tertiary pediatric nursing and IV therapy and central venous catheter care and management, including one year in an educational or leadership role, or an equivalent combination of education, training and experience.
•    CPR Training – Basic Cardiac Life Support (BCLS) – Level I.
•    Eligible for the Canadian Vascular Access Association (CVAA) certification.
•    Valid BC Driver's License and access to personal vehicle for travel outside of existing geographic area may be required, and will be compensated in accordance with applicable collective agreement language.

Skills and Abilities

•    Knowledge of community based OPAT services for children and youth.
•    Comprehensive knowledge of the British Columbia College Nurses & Midwives (BCCNM) standards for Nursing Practice.
•    Comprehensive skill and expertise in tertiary pediatric services and related issues
•    Demonstrated knowledge of current practice approaches and “Best Practice” standards in the delivery of outpatient IV therapy and vascular access in the pediatric population, including care, repair/maintenance and troubleshooting of lines, equipment and other parts.
•    Demonstrated ability to teach, demonstrate, facilitate and lead.
•    Excellent communication skills, both oral and written.
•    Strong leadership and organizational skills.
•    Ability to relate and communicate effectively to children/youth and their families, dealing effectively with those experiencing stress and /or in crisis.
•    Ability to collaborate and work effectively as a member of an interdisciplinary care team and function as a nursing resource.
•    Ability to identify and resolve problems using therapeutic communication strategies.
•    Ability to work independently to organize and prioritize multiple tasks.
•    Physical ability to carry out the duties of the position.
•    Ability to operate related equipment, including computers.
•    Ability to identify and assess learning needs and develop educational programs.
•    Ability to deal effectively with a variety of internal and external contacts, including community agencies, representatives, patients and families.
