FlexStaff is seeking a Nurse Case Manager/Staff Nurse for a 13 week contract with our client, a leading organization that provides housing, services and programs for individuals with intellectual and developmental disabilities. 

This position will be covering residential group homes in Yonkers, Garrison, Carmel 

Schedule: Monday-Friday, 9am-5pm

Contract Pay Rate: $80/hr 

The Nurse Case Manager will coordinate the delivery of health care services for persons receiving services residing in a residential facility.

Responsibilities

Coordinates the delivery of routine and specialty health care services for adults with disabilities residing in residential programs Assesses and establishes health care outcomes for persons receiving services. Updates assessments on at least a semi annual basis or as indicated. Maintains health care profiles. Provides guidance and training to direct care staff to ensure proper delivery of health care services. Develops plan of nursing services and oversees the implementation of the health care plan by educating person’s receiving services, DSP/Caregivers and families in the plan of nursing services. Evaluates the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary. Acts as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providers Completes relevant medical documents, assists in developing policies, procedures and forms; and establishing working systems.

Requirements

NYS RN License, BSN preferred. Prior nursing experience and/or experience working with persons with developmental disabilities preferred. Excellent oral and written communication skills..
