As a Nurse Care Manager, you'll be responsible for providing comprehensive care management services, including assessing client needs, developing treatment plans, and coordinating with healthcare providers. Key Responsibilities:· Develop wellness and prevention initiatives to promote client health and wellbeing
· Facilitate health education groups using advanced knowledge of complex medical conditions
· Collaborate with treatment team members to develop and implement individualized treatment plans
· Manage hospital and medical provider admissions and discharges for clients
· Train patients on complex health conditions, medical diagnosis, treatment, and medications
· Implement and utilize technology programs and initiatives to improve patient outcomes
· Monitor and report performance measures and outcomes using sophisticated information systems

Education, Experience, and/or Credential Qualifications:· Active RN license
· Experience working with the IDD population preferred
· Knowledge of chronic disease processes in children and adults
· Excellent written and verbal communication skills

Additional Qualifications:· Current driver's license, acceptable driving record, and current auto insurance
· Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint check

Physical Requirements:· Sedentary work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently to lift, carry, push, or pull objects
· Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift
· Sedentary work involves sitting most of the time, with occasional walking or standing for brief periods
