Provide education and support to clients with complex medical conditionCollaborate with internal and external treatment teams to develop personalized treatment plansFacilitate health education groups and wellness initiativesParticipate in treatment plan development and implementationKey Responsibilities:
Develop and implement personalized treatment plans in collaboration with internal and external treatment teamsProvide education and support to clients with complex medical conditions, facilitating their wellness and prevention initiativesFacilitate health education groups and wellness initiatives, promoting healthy lifestyle choicesParticipate in treatment plan development and implementation, ensuring comprehensive careCollaborate with treatment team members to identify and address client needsEducation, Experience, and/or Credential Qualifications:
Active RN licenseExperience with the IDD population preferredAdditional Qualifications:
Excellent written and verbal communication skills
Ability to implement new innovative systems
Ability to make decisions based on critical thinking and nursing process
Successful completion of background check, including criminal record, driving record, abuse/neglect, and fingerprint check
Current driver's license, acceptable driving record, and current auto insurance
Physical Requirements:
