**Schedule:**
PRN, rotating schedule
**_This is a regional position out of our Newtown Square System Office. Training at this location will be three to six months, then will be remote._**
The Registered Nurse working in the THMA Centralized Bed Planning/Transfer Center must work well independently and within a team. This RN must accurately listen and respond appropriately to requests made by hospital-based colleagues, transferring physicians, and peers. He or she will be responsible to ensure patients are assigned rooms in an expedient manner utilizing all critical resources within the organization as well as critical thinking to expedite this process. This position gives high priority to customer service, safe and efficient care delivery, communicating efficiently and effectively with the surgical services, emergency department, trauma service, nursing units, physician offices, bed schedulers, and outlying facilities to ensure appropriate placement of patients. This RN must be able to multitask and adjust to a rapidly changing environment. This position may be required to work weekends, holidays, nights and be on call if scheduled.
+ As the RN you will facilitate patient throughput with an ongoing focus on quality and efficiency, providing follow up to all participants of the patient throughput process. You will ensure timely, accurate and concurrent data collection and maintenance in both EPIC and other systems as appropriate to allow for aligned bed assignment, and provide pertinent patient information to assist with the development of an initial plan of care.
+ The RN will be responsible for collecting clinical information relative to a bed placement and/or a transfer request and then facilitates the process by matching the patient’s clinical needs to the facility with bed capacity and service capability and appropriate bed placement within the facility in accordance with the current Transfer Center processes.
+ As the Nurse - Admission/Placement in Centralized Bed Planning, you will use acquired nursing knowledge and experience to facilitate the transfer and acceptance of patients for all facilities within the markets served and perform Bed Planning and Transfer Call Center functions.
+ This RN will coordinate transfer of emergent, urgent and elective patients to a final destination facility from all potential referral sources to include but not limited to hospitals, clinics, Nursing Homes, and physician offices to initiate acceptance and expedite decision making relative to a potential transfer.
**Requirements**
+ BSN from an accredited College or University
+ BLS and ACLS Certification
+ **_Active Nursing license in the state of Pennsylvania and Delaware upon hire_**
+ 2-4 years previous experience required
+ 5-8 years previous experience preferred
+ Knowledge of utilization management i.e. acuity.
+ Ability to define appropriate level of care preferred
+ Proficient with computer skills.
+ Excellent time management skills required
+ Proficient in bed management
**Preferred Experience includes: Bed Planning, Case Management, Leadership, Transfer Center, and Utilization Management**
