Job Description
The Clinical Case Manager plays a crucial role in coordinating all systems and services required for a well-organized, multidisciplinary, patient-centered team approach. This position ensures cost-effective care for Managed Medi-Cal patients by managing the course of treatment and coordinating with physicians, nurses, case managers, and other staff.
ResponsibilitiesCoordinate all systems and services needed for a multidisciplinary team approach.Manage the course of treatment for patients.Coordinate care with physicians, nurses, case managers, and other staff.Ensure cost-effective care for Managed Medi-Cal patients.Essential SkillsProficient in healthcare practices.Experience with prior authorization processes.Expertise in case management.Knowledge of Skilled Nursing Facilities (SNF).Additional Skills & QualificationsPossession of a valid CA LVN License.Work Environment
This position offers a remote work arrangement with a Monday-Friday day shift. The office setting at the corporate location is equipped with dual screens to facilitate efficient workflow.
Pay and Benefits
The pay range for this position is $38.00 - $43.00/hr.
