Job Title: Nurse Job Description As a team lead, you will respond to complex medical issues, ensuring consistency in benefit application and may lead cross-functional teams, projects, and initiatives. You will also serve as a proctor to new hires, participating in training and retraining of lower-level nurses. Additional duties include assisting with the development of care management policies, chairing meetings, presenting cases for discussion at Rounds/Care Conferences, and participating in interdepartmental workgroups. You will manage a continuum of care from admission through discharge for assigned patients, partnering with medical staff and utilizing scientific evidence for best practices. Responsibilities + Manage a continuum of care from admission through discharge for assigned patients, partnering with medical staff to ensure compliance with mission, standards, and core values. + Proactively assess and manage all high-risk, high-volume cases throughout the continuum of healthcare needs or until maintenance mode is reached. + Demonstrate a multidisciplinary approach in identifying problems and negotiating with the member’s primary care provider, family members, caregivers, healthcare team, community resources, and health plans. + Participate in the development of patient-focused long and short-term goals, implementing, evaluating, and modifying the care plan. + Research and refer members to alternative funding sources, community services, and other support services for patients and families. + Track all CCS referrals for timely notification of decisions and assist the Medical Director with CCS appeals. The work shift is Monday to Friday, from 7 am to 3:30 pm. The role requires 40% travel within the Sacramento area, specifically onsite at designated locations, two days per week.
