Position Overview
PACT is an existing program operating as a part of the Shared Health organization that provides wrap- around interdisciplinary services and supports to individuals who have symptoms of a severe and enduring mental illness. The community PACT teams work with individuals who have a severe mental illness and may have been in hospital for extended periods of time, have a high use of emergency services, use a number of services and require regular supports to assist in community living. Individuals do require a diagnosis of primary psychotic disorder (Schizophrenia, Schizoaffective, Bipolar I), but may also have co-existing medical conditions, addictions, or be involved with the criminal justice system.
The model is an assertive community treatment approach and allows for a coordinated care approach within the interdisciplinary team.
PACT will be responsible for:
case management services in the community. serving as a liaison point for program participants linking community partners. including families and natural supports as indicated in care plans.
The PACT team would allow for intensive services and supports for individuals with severe mental illness and may have a number of functional and adaptive needs. The PACT team will provide 24-hour wrap around supports provided by an interdisciplinary team which includes after hours on call support.
Main Function:
Under the general supervision of the PACT Team Leader the nurse responsible for providing case management services and supports to individuals involved with the PACT team. They are also responsible for coordinating medical services and medication management for Individuals receiving PACT services.
The nurse is responsible for assessing, planning, developing, and coordinating treatment, rehabilitation along with support services provided to individuals by PACT. Under the clinical supervision of the team leader and psychiatrist. The nurse will provide services to individuals using the principles of psychosocial rehabilitation and recovery to guide their practice and function as a clinical member of the interdisciplinary team. The nurse will be responsible for documentation, liaising with pharmacy, medication education to individuals/family/natural supports, administration of oral medications along with long acting injections in the home or the community that are prescribed by the PACT psychiatrist or other health prescriber. The nurse will also provide crisis on call services.
Experience Minimum four years related community experience working with people who have serious and enduring psychiatric illness’; combination of institution and community experience will be considered. Education (Degree/Diploma/Certificate) Nursing degree required. BScMH, Bsc.PN, BN, RPN, RN. Certification/Licensure/Registration Registered with the College of Nurses of Manitoba. Registered with the College of Registered Psychiatric Nurses of Manitoba. Qualifications and Skills Knowledge of: Relevant legislated acts including but not limited to Personal Health Information Act, Protection of Persons in Care Act, Mental Health Act, Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices (Universal Precautions). Regional/site/unit policies, procedures, protocols, and guidelines relating to clinical care. Working with individuals from diverse cultural, social, and ethnic backgrounds. Working knowledge of treatment, rehabilitation and support in mental health. Skills in assessing, planning, development and coordination of support services. Working knowledge and experience in working with people experiencing addictions. Subject to a criminal record check. Physical Requirements Valid driver’s license and vehicle required.
This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.
