Pay Range: $19.25 / hour

Additional components of compensation may include:

Evening, night, and weekend differential Overtime On-call pay

POSITION SUMMARY:

The Medication History Technician helps to optimize the medication history process and decrease the number of medication errors due to incomplete or inaccurate information.  Also helps to improve medication safety by assisting pharmacists in collecting up to date medication lists for patients being admitted to the hospital. Also assists in optimizing patient outcomes through obtaining a complete medication history for patients admitted to the health system as well as updating and clarifying allergy information in patients' electronic health record.

EDUCATION/TRAINING:

Minimum:

High school education, or a GED

Preferred:

Any advanced Pharmacy Technician education

LICENSURE/CERTIFICATION:

Minimum:

Completion of Pharmacy Technician Certification within two (2) years of date license was first issued Current licensure from the State of Illinois. Candidates without a license may work in this role for 60 days from date of license application through Illinois Department of Professional and Financial Regulation

REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES:

Hospital and Outpatient Pharmacy knowledge, including, drug name knowledge, dosage forms, and indication for use.

 

Additional training in customer service skills, communication skills, and various software.

 

MAJOR RESPONSIBILITIES:

Utilizing great customer service skills, while talking with patients and/or their caregivers, physicians, and pharmacy staff.

 

Must have the ability to utilize multiple software systems.

 

Generate a current accurate medication history for each patient who is admitted to the hospital.

 

Enter into the computer the accurate medication history including updating current medications if necessary.

 

Work closely with the clinical pharmacist.

 

Time management skills required to work independently.

 

Must have excellent communication skills and present in a professional manner.
