POSITION SUMMARY:

Serves as an advocate for associates of UW Health by providing clinical and primary nursing care and case management services to associates whose capacity to perform their expected duties is impaired or at risk of being impaired due to injury or illness.  Provides medical services including first aid and pre-employment, workers' compensation injury/illness, occupational health assessment, treatment and case management, workers' compensation mechanism of injury investigations.

EDUCATION/TRAINING:

Minimum:

Bachelor of Science in Nursing or Associate degree with equivalent experience.

LICENSURE/CERTIFICATION:

Minimum:

Current Registered Nurse licensure in the State of Illinois.

Bi-Annual BLS.

Preferred:

Certified Occupational Health Nurse (COHN) designation.

EXPERIENCE:

Minimum:      

4 years clinical experience in direct patient care.

Preferred:

N/A

MAJOR RESPONSIBILITIES:

In conjunction with other members of Associate Health, works to improve the quality of associate care and ensure the efficient and cost effective movement of associates through the health care continuum.

 

Conducts individualized assessment of associates’ needs to develop a comprehensive case management plan. Plans specific objectives and actions based on the assessment. Implements the plan to achieve stated objectives. Evaluates the plan at appropriate intervals to ensure that desired outcomes are reached.

 

Coordinates various resources necessary to accomplish established goals. Employs negotiation skills as necessary to obtain the resources needed to meet the established goals.

 

Demonstrates compliance with laws, regulations and standards governing health and safety for associates and the environment.

 

Investigates, monitors and analyzes illness and injury episodes and trends, as well as methods to promote and protect employee health and safety.

 

Consistently employs illness prevention, health promotion, and maintenance and restoration strategies.

 

Coordinates associate health services on a daily basis to include post-offer physical examinations, immediate care for illness or injury, regulatory compliance and health promotion.

 

Demonstrates effective documentation and communication skills in recording patient care notes and other health records and reports as well.

 

Promotes safety and infection control procedures.

 

Conducts Workers’ Compensation contract tracing, fit for duty evaluations, chart prep diagnostics, therapy, referrals and office visits reports, physicals, drug and alcohol screenings and evaluations of associates returning from leaves.

 

Reviews OSHA reports to ensure the organization meets occupational health safety legislation.

 

Completes quarterly safety reports.

 

Facilitates completion of accident reports after investigation and makes recommendations for prevention.

 

Coordinates a holistic, multidisciplinary approach to delivery of safe, quality. Comprehensive associate health and safety services.

 

Performs other duties as assigned.
