Musculoskeletal Athletic Trainer Certified Navigator

Job Description:

The MSK navigator collaborates with multidisciplinary teams in the ambulatory care setting, working directly with patients to support the patient’s care plan to help assist them in providing cost effective and comprehensive care for MSK and pain related conditions. The MSK ATC Navigator plays a key role in managing and triaging incoming orthopedic, sports, and pain referrals, coordinating patient care across the MSK and Medical Specialties service lines, and improving patient access and experience. This individual serves as the first point of clinical contact for patients and referring providers, ensuring efficient routing to appropriate providers or services based on injury or condition severity.



Essential Functions

Triage and route referrals by reviewing all incoming orthopedic/musculoskeletal and pain referrals, and determine appropriate provider or specialty level.

Clarify clinical details by communicating with referring providers and patients to confirm symptoms, imaging, and/or medical history.

Coordinate timely care by prioritizing urgent/complex cases and ensuring timely scheduling or escalation.

Conduct care coordination for patients needing further assistance, including gathering in-depth medical, functional, and social background.

Facilitate patient/family goal setting by collaborating with patients and caregivers to identify health goals and action plans.

Support care plan implementation by collaborating with multidisciplinary teams and promoting patient self-management strategies.

Promote resource access by working with community partners to improve access and utilization of healthcare and social services.

Educate and support patients and staff regarding case management, care planning, and available resources, including social determinants of health.

Maintain efficient service delivery by monitoring service availability and making adjustments to meet patient needs.

Skills

Strong clinical triage and assessment skills

Effective interdisciplinary communication (verbal & written)

Ability to coordinate complex care plans and navigate systems

Strong knowledge of orthopedic, musculoskeletal and pain care pathways

Proficient in identifying and addressing social determinants of health

Experience with patient education and promoting self-management

Ability to build rapport with patients, caregivers, and clinical teams

Knowledge of community and healthcare system resources

Organization and time management skills for handling multiple referrals

Familiarity with electronic health records and care documentation

Qualifications

Bachelor’s degree in athletic training or related field

Licensed or certified Athletic Trainer (ATC/LAT) in the state of employment

Clinical experience in sports medicine, orthopedics, spine/pain, or rehabilitation settings

Related working experience, preferred

Physical Requirements:

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information and needs.Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment.May be expected to stand in a stationary position for an extended period of time.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

$29.14 - $44.94
