Job Description
We are seeking a dedicated MTM Pharmacist to perform comprehensive medication therapy reviews with eligible members. The role involves reviewing medication profiles to identify, prevent, and resolve medication-related issues, thus maximizing member benefits and minimizing costs. You will document member encounters in the MTM platform and engage with other healthcare professionals as necessary. Additionally, you will conduct sessions with members to enhance medication adherence, address therapy gaps, and improve overall health outcomes.
ResponsibilitiesPerform comprehensive medication therapy reviews with eligible members.Review member’s medication profiles to identify, prevent, and resolve medication-related problems.Document member encounters in the MTM platform.Conduct sessions to improve medication adherence and resolve therapy gaps.Complete outreach to other healthcare professionals as necessary.Contribute to projects to support business needs.Collaborate with team members in MTM/CCM support roles.Other duties as assigned.Essential SkillsMTM and ambulatory care experience.Minimum of 1 year of experience as a licensed pharmacist.Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy Degree.Current pharmacist license in good standing with the State Board of Pharmacy.Work Environment
This is a remote position with flexible hours, typically between 8 a.m. to 5 p.m. CST, with potential for extended hours until 6 p.m. CST. Candidates can reside in any U.S. state except California and Hawaii, although they can be licensed in any state. New pharmacists are considered potential long-term employees, with contracts evaluated every 6 months for extension. Training lasts approximately 3 weeks, with minimal time off during the initial 2 weeks, to ensure a smooth acclimation and setup with equipment and team interactions.
Pay and BenefitsThe pay range for this position is $55.00 - $55.00/hr.
