As a Community Support Specialist, you'll help individuals adapt to living in their communities by creating personalized plans that address their specific needs. Key Responsibilities:
Provide community-based mental health services including assessment, diagnosis, treatment planning, and intervention.
Collaborate with clients, families, and stakeholders to develop individualized treatment plans.
Utilize evidence-based practices such as Motivational Interviewing, CBT, DBT, and Trauma-Informed Strategies.
Deliver behavioral interventions including operant conditioning, group dynamics, and cognitive processing.
Coordinate care with service providers including physicians, psychiatrists, schools, and community organizations.
Maintain accurate and timely documentation.
Participate in professional development and training.
Support quality improvement initiatives.
Work collaboratively with internal teams.
Build relationships with key stakeholders.
Facilitate PSR group sessions aligned with treatment goals.
Perform other duties as assigned.
Demonstrate knowledge of ACT model principles (ACT/ACT TAY/IMPART only).
Education and/or Experience Qualifications:
Bachelor’s degree in Psychology, Social Work, or related field.
Bachelor’s degree in unrelated field with two years of relevant experience.
Four-year combination of higher education and two years of related experience.
Associate of Applied Science in Behavioral Health Support.
Four years of qualifying experience in mental health, substance use, or developmental disabilities.
Required License/Certification (one of the following):
High school diploma with one year of social services experience (RSA designation).
Bachelor’s degree in human services or other field with two years of supervised clinical experience.
Practical nurse license under Illinois Nurse Practice Act.
DHS-approved psychiatric rehabilitation certificate.
Certified Recovery Support Specialist or Family Partnership Professional.
Licensed Occupational Therapy Assistant with one year of mental health experience.
High school diploma/GED with five years of supervised clinical experience.
Additional Qualifications:
Knowledge of case management and rehabilitation techniques.
Familiarity with community resources and legal procedures.
Strong verbal and written communication skills.
Self-motivated and able to meet complex client needs.
Effective multitasking and coordination across services.
Ability to build rapport with youth and families.
Receptive to supervision and direction.
