Summary:
Fill orders with accuracy; check technician prepared products before release; maintain security of pharmacy and controlled substances; enter orders and patient information with accuracy; aid in the training of new staff; update the Director about the daily activities of the pharmacy; aid the Director in maintaining the operations of the pharmacy.
Responsibilities:
Enters correct charges for all medications
Enters correct medication including description strength route expiration date and start/stop dates
Enters mg/kg dosing for peds/neonates. Refers all pediatrics orders for doublecheck
Corrects all orders which are entered in error or may be misunderstood
Posts debits/credits accurately
Follows all ATS conversions based on substitutions approved by P&T
Maintains order entry error rate <2%
Enters all patient demographic information as needed
Completes work assignments within an acceptable time frame
Generates required reports
Alerts Director of any problems with system software or hardware
Assists technicians in performing technical tasks
Accurately completes IV process
Maintains aseptic technique
Recycles products as appropriate
Labels containers accurately including auxiliary labels
Prioritizes workload
Assists Tech in restocking shelves and rotating stock
Disposes of all waste properly
Checks technician prepared products prior to release
Enters correct patient data for all TPN's via the intranet
Clarifies all TPN orders prior to order entry
Performs all calculations and worksheets for chemotherapy
Completes all documentation and double checks
Takes required protective measures and ensures that technicians take all required protective measures
Dispose of chemotherapy waste properly
Prepares and dispenses drug products throughout the hospital (or directly supervises qualified pharmacy technician in these functions)
Ensures the appropriateness of drug dose and frequency for each medication ordered before dispensing. Clarifies any ambiguities with prescriber
Reviews all physician orders for appropriateness
Prepares labels and dispenses all drug products using appropriate technique
Fills all orders with correct medication
Checks support staff accuracy prior to medication delivery
Prioritizes work
Checks all medications for delivery to nursing floors in a timely fashion
Oversees daily technician activities to maintain an effective workflow
Maintains records as required by law to document drug transactions within the Pharmacy and throughout the hospital
Controlled drug sign-out sheets are initiated correctly and terminated correctly when new supplies issued
Investigational drug logs and perpetual inventories are accurate and current
Completes all Controlled Substance logs and paperwork appropriately
Maintains required continuing education and gives copies to management
Checks that all batch logs are completed accurately
Follows non-formulary drug procedures
Completes medication error reports as needed
Orders medications when stock is low
Loans and borrows medications per policy
Safely uses departmental equipment
Dispose of waste in proper containers
Is able to locate the MSDS sheets disaster manual safety manual infection control P&P etc
Demonstrates fire safety verbally
Assists in training of new staff
Helps in other areas of pharmacy as needed
Is able to refill check and billg Emergency Med Carts TPA kits etc
Maintains security of pharmacy
Maintains security of controlled substances
Supervises activities of support staff effectively
Provides Director with feedback regarding activities of support staff
Work with other pharmacists and technicians to settle work priority disputes
Reviews patient profile to ensure appropriate and safe drug therapy
No therapeutic duplication
Prevents significant drug interactions
Prevents avoidable ADR secondary to allergies. Updates allergies on order enter
Medication ordered is appropriate for patient height weight age renal function and disease state
Serves as a hospital resource for drug information including indications doses routes side effects adverse reactions drug interactions pharmacokinetics and product availability. Able to utilize appropriate reference materials when necessary
Participates in drug inventory control through formulary management performing therapeutic interchanges per policy and purchasing
Responsible for maintaining the pharmceutical quality assurance standards
Provides assistance to the Director of Pharmacy in joining specified committees and participating in activities of the hospital when requested
Provide input/ suggestions to improve the pharmaceutical services provided
Is familiar with and follows Federal and State laws regarding the profession of pharmacy
Understands pharmaceutical and chemical procedures and techniques and weights and measures in all systems
Understands the role of the pharmacy department in the hospital and its relationship with other departments
Generates reports and completes all clinical functions related to reports such as Renal dosing IV to PO Anesthesia audits and any other pertinent reports
Informs oncoming shift of patients or issues requiring monitoring or follow up
Keep current on new drugs and know where to access information
Continuously assesses and improves the performance of care and services provided. Offers creative solutions or alternatives to issues or concerns
Uses systematic methods to improve services plan changes and evaluate outcomes
Demonstrates effective problem solving techniques and promotes same in others
Maintains awareness actively supports the mission goals and objective of the hospital and department
Completes special projects on time
Is part of a team effort to ensure quality and safe services
Produces quality results
Accepts responsibility for all work performed and takes appropriate corrective action as needed
Follows the Joint Commission guidelines

Other information:
Education:
Graduate of a college of pharmacy accredited by the American Council on Education (ACPE). Doctor of Pharmacy degree preferred.
Current MA Pharmacist Licensure in good standing.
Experience:
One to two years of hospital pharmacy experience preferred
Hospital pharmacy residency preferred
Very good drug knowledge base including pharmcokinetics dosing and nutritional support
Working computer knowledge
Effective communication skills verbal and written
Ability to work independently and handle stress approrpriately
Ability to work cooperatively with other memebers of hospital and pharmacy staff
Ability to handle multiple tasks and adapt to changes in workload and work schedule
Ability to set priorities makes critical decisions and responds to client concerns
