**Position Summary:**
Join our compassionate team delivering high-quality, patient-centered care in a home-based setting. As a Primary Care Nurse, you’ll conduct initial assessments, develop care plans in collaboration with physicians and case managers, and provide ongoing therapeutic interventions. You’ll also supervise LPNs and Home Health Aides, ensuring top-tier outcomes and patient satisfaction.
**Key Responsibilities:**
+ Perform comprehensive patient assessments and develop individualized care plans
+ Deliver nursing care per physician orders and agency protocols
+ Monitor patient progress and adjust care plans as needed
+ Utilize motivational interviewing and health coaching techniques
+ Document care accurately and timely in EMR systems
+ Collaborate with interdisciplinary teams to optimize patient outcomes
+ Mentor and support dependent disciplines
**Qualifications:**
+ RN licensure in state of practice; BSN preferred
+ Minimum 1 year of professional nursing experience (Home Health preferred)
+ Strong technology and documentation skills
+ Valid driver’s license and reliable transportation
+ Commitment to ethical standards and organizational mission
