As a Mental Health and Addictions Nurse (MHAN), you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

What will you do?

Ensuring effective support for the Mental Health student population to ensure communication and linkage with longer-term community support, primary care and providing timely and effective Mental Health nursing in the school/communityProviding the initial contact within 72h from referral onset as well as the first in-home nursing visit within seven (7) days from hospital discharge or community referral. During this visit, the nurse will confirm the patient hospital discharge care plan, safety planning, goals of care and at times, medication reconciliation for the client.Assessing mental health statusProviding ongoing risk assessment and safety planning for crisis interventionMonitoring medication managementLiaising with attending physicians, psychiatrists and other support servicesProviding education and supportive counselingAssessing physical health and providing clinical nursing care where appropriateProviding education regarding mental health, medication and coping strategies to patients, family and caregivers as well as school staff

 

What must you have?

3 – 5 years of relevant recent experience as a Registered Nurse in Mental HealthCNA Mental Health Certificate, or a comprehensive Mental Health Education Program that has been reviewed and approved by Ontario Health atHome Waterloo Wellington in writingRegistered Nurse (BScN or diploma) Certification in mental health nursingProficient in the operation of a personal computer in a networked environment using a variety of computer software and data entry skills, i.e. Excel, Data Base Software, as well as various other programsKnowledgeable in the use of technology in a community setting – e.g., lap top in the home, iPhone, remote networking, etc.Demonstrated skill in setting and meeting deadlinesDemonstrated effective use of time and tools to manage work efficientlyDemonstrated experience in comprehensive assessment, use of different or combined approachesDemonstrated skills in synthesizing information in order to plan and adjust services and to assist clients with informed decision-makingDemonstrated skill in crisis management and risk situationsRequires analysis and critical thinking skillsWorking knowledge of community resources and roles of health care professionalsSolid knowledge of health care related legislation and practicesKnowledge of direct care/case management models used in community health care organizationsKnowledge of Ontario Health atHome priorities, policies, practices and service standardsEffective interpersonal and communication skillsEffective organizational and planning skillsBasic proficiency with computerized information systemsEffectively maintain a constant flow of verbal and written communication with others throughout the workplace as well as outside the organizationAble to communicate with clients’, their families, and other relevant individuals in order to follow through with care plan directivesDemonstrated awareness of cultural diversity, as well as ability to behave discreetly and sensitively to confidential issuesA valid driver’s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID-19 vaccination policy.
