We are seeking a strategic and collaborative leader to drive forward our service integration agenda. In this pivotal role, you will foster stronger partnerships between CGL services and external stakeholders across integrated care systems and Combatting Drugs Partnerships. Your work will directly support the delivery of the national Drug Strategy, helping services respond effectively to health and social care challenges.

 

You will guide and support service leaders, offering expert advice to ensure our services are aligned with local system priorities and community needs. By strengthening integration with criminal justice and health and social care providers, you will help improve treatment pathways, reduce substance-related harms, and contribute to tackling health inequalities.

Hours: Full Time. 37.5 per week

Full Time Salary: £39,891.91 - £46,683.60 dependent on experience (based on full time hours, pro rata fo part time)

Allowances: £4,133.14 Inner London Weighting (based on full time hours, pro rata fo part time)

 

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Key Responsibilities:

Manage relationships with a significant number of Mental Health services, including Mental Health Supported Accommodation and in-patient wards.Cultivate strong connections with GPs, Primary Care Networks, and two large Acute Trusts (Royal Free and UCLH).Identify areas for integrated working, developing robust pathways and processes for reviewing referrals, and resolving issues/challenges.Represent the Substance Misuse service at various operational meetings.Collaborate with internal teams to ensure effective service integration strategies.

Requirements:

Proven experience in managing partnerships with Mental Health services, GPs, Primary Care Networks, and Acute Trusts.Strong understanding of Substance Misuse needs and referral processes.Ability to identify opportunities for integrated working and develop effective pathways.Skilled in resolving issues and challenges related to referrals.Representation experience at operational meetings.Excellent communication and relationship-building skills.
