MEDICATION HISTORIAN TECHNICIAN
BOISE REGIONAL MEDICAL CENTER
SUMMARY:
Assists with the accuracy and completeness of medication lists for patients across the continuum of care beginning in the Emergency Department or the inpatient units and under the direct supervision of the Pharmacy Department.
REQUIREMENTS:
Must be licensed as a Certified Pharmacy Technician with the Idaho State Board of Pharmacy.
Minimum 3 years of experience as a pharmacy technician required, preferred to have 6 months with Saint Alphonsus
National Certification (CPhT, ExCPT or equivalent) required.
Knowledge of community pharmacy operations preferred.
WHAT YOU WILL DO:
Gathers information about the patient's medications from family or directly from the patient in a manner that is professional and non-invasive.
Works independently to perform medication list confirmation for patients being admitted to the hospital and compiles a complete list of patient's home medications including over the counter drugs and herbal supplements including drug name, dosage, and frequency.
Coordinates with nursing, pharmacy, and providers to ensure medication lists are updated within 24 hours of admission.
Interviews to obtain any known drug allergies and reactions.
Familiar with the outpatient dispensing models and pharmacies patients may be engaged with
Documents tasks performed within the electronic medical record.
Follows up with community pharmacies and physician offices as required.
May be required to function as staff pharmacy technician if the need arises.
