Description:Job Title: Social Worker – Homecare/Hospice

Summary

Delivers medical social services to homecare and hospice clients under physician orders and supervision of a Master’s-level Social Worker. Supports clients and families in managing social, emotional, and environmental challenges related to illness and care.

QualificationsBachelor’s in Social Work (BSW) from an accredited program1+ year of healthcare social work experienceState registration/licensure (as required)Valid driver’s license and reliable transportationAbility to work under supervision and within interdisciplinary teamsKey ResponsibilitiesAssess client needs and develop care plansProvide counseling and connect clients to community resourcesCollaborate with care teams and participate in treatment planningDocument care activities accurately and timelyMaintain compliance with regulations and agency policiesSupport quality improvement and professional development initiativesWork RequirementsFrequent travel to client homesAbility to lift, stand, and perform physical tasks as neededAdapt to changing priorities and work environmentspay grade range- $31.73-$50.77
