Key Responsibilities:

Complete and manage MDS assessments and care planning
 Collaborate with interdisciplinary teams to ensure compliance and accuracy
 Monitor documentation to optimize reimbursement and resident outcomes
 Ensure compliance with all state and federal regulations
 

Qualifications:

Licensed RN or LPN in the state of Ohio
 Previous MDS experience preferred
 Strong attention to detail and organizational skills
 Effective communication and teamwork abilities
