Job Title: MDS Nurse, LPN
Department: Long Term Care                    
Location: DeMay Living Center                        
Hours Per Week: 40
Schedule: Days, Monday-Friday 8a-4:30p                        
 

SUMMARY:

Completes the MDS

Responsibilities:
• Completes the nursing portion of MDSs within the time frame specified.
• Reviews clinical documentation as well as Care tracker to complete the MDS
• Assist in setting MDS dates and publish to all clinical disciplines as needed
• Assist in educating clinical staff about the MDS process and documentation
• Knowledgeable in the use of windows based software applications, good communication skills. Ability to work as a team member and to be a resource for clinical staff.
• Completes other dues as assigned

REQUIRED QUALIFICATIONS:
• Graduate from an accredited School of Practical Nursing

PREFERRED QUALIFICATIONS:

• Preferably has attended MDS seminars, has received certification in MDS competency, or is willing to obtain certification.
• Experience in Long Term Care preferred.

EDUCATION:

LICENSES / CERTIFICATIONS: 

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
