Graduate of an accredited School of Nursing
CPR certification
Minimum 2 years of nursing experience, preferably in a rehab or skilled setting
Prior MDS experience and PointClickCare proficiency
Familiarity with CMS RAI guidelines strongly preferred
Strong communication, organizational, and leadership skills
Contribute to high-quality resident outcomes and regulatory compliance

Are you an experienced RN passionate about clinical excellence, team leadership, and ensuring accurate reimbursement? We’re seeking a skilled MDS Coordinator RN to lead the Resident Assessment Instrument (RAI) process at our sub-acute rehab facility in Grand Rapids, Michigan.
Position Highlights:
As the MDS Coordinator RN, you will:

What We're Looking For:
Current RN license in Michigan (required)Graduate of an accredited School of Nursing
