\nThe Manager Comprehensive Care Management in Social Work oversees activities and staff. They assess and coordinate patient care across the continuum. This includes case management, social work, utilization review, and care coordination. The goal is to achieve optimal clinical and quality outcomes. They manage care and resources to reduce unnecessary utilization.

\nEssential Functions of the Role
\n\nProvides administrative, technical, and clinical management of assigned staff. This includes hiring, firing, performance evaluation, education, staff development, work allocation, and problem resolution.\nEnsures patient care needs are met and treatment plans are executed properly. Coordinates and helps solve problems related to care coordination, discharge planning, case management, and utilization review. Monitors processes and systems for efficient service delivery. Establishes tools, processes, and systems to optimize case management.\nDevelops structures, relationships, and processes to help physicians, pharmacists, nurses, social workers, and other health professionals. This facilitates participation in multidisciplinary care planning, care delivery, and discharge planning.\nEnsures appropriate scope of practice by staff. Ensures subordinates know key ethical standards like confidentiality, informed consent, conflicts of interest, dual relationships, boundary issues, documentation, consultation, referral, and end of services.\nDevelops meaningful performance measures to assess the quality, effectiveness, and efficiency of operations. Provides administrative, technical, and clinical management of assigned staff under industry best practices. This includes hiring, firing, performance evaluation, education, staff development, work allocation, and problem resolution.\nDevelops and implements standardized policies for efficient operations and optimal patient care. Ensures compliance with institutional policies and regulatory requirements.\n\nKey Success Factors
