Job Qualifications
· 5 years’ experience in health care, preferably home health care & hospital.
· Current license in good standing as a Registered Practical Nurse, in the province of Ontario.
· Graduate of an accredited and approved program.
· Completed Continuing Education Requirements (CEUs) as required by regulation
· Solid knowledge of the principles, practices, and methods of community nursing and service delivery.
· Knowledge of Ontario community health system navigation and patient discharge care planning recommended.
· Exceptional business acumen and ability to collaborate with hospital partners, community partners and stakeholders at all levels.
· Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format
· Exemplifies characteristics of Bayshore Health Care\: compassion, excellence and reliability.
· RAI Tool experience and knowledge an asset.
· Evidence of current CPR certification
· Have valid driver’s license and reliable vehicle for commuting
· Knowledge of health & safety and infection prevention and control.
· Demonstrated leadership skills
