*Description* Manager Behavioral Health- Primary Care LCSW Opportunity Summary: Manage the growth of the integrated care program. Expand program to Nuvance West. Maintain, create and modify general workflows PC BHC role as well as SBIRT, MAT, Metabolic/ Bariatric, Endocrinology, Residency, Surgical Cardiology, and other specialty programs. Monitor RVU’s for budget management for BHC staff monthly. Responsibilities: 1.Manage clinical face time with patients 2-3 days a week. Supervise and train BHC staff individually. 2.Attend staff and provider meetings for Southbury PCP office. 3.Supervise and train BHC staff individually. 4.Maintain, create and modify general workflows for PC BHC role as well as SBIRT, MAT, Metabolic/ Bariatric, Endocrinology, Residency, Surgical Cardiology, and other specialty programs. 5.Respond to staff daily for clinical questions and coverage for staff when on PTO. 6. Audit 2 charts per all staff per month and SBIRT data collection sheets monthly. 7.Maintain data collection sheets for SBIRT and provides Sim Lab participation. 8.Monitor RVU for budget management for BHC staff monthly. Facilitate monthly staff meeting. 9. Ensure department regulatory compliance with standards through policies and practices. Responsible for the quality of services provided including identification of performance improvement and patient safety opportunities, in addition to developing, implementing, monitoring and evaluating quality and safety initiatives. 10.Meet weekly with SBIRT grant coordinator, and with health coach for supervision and development. 11.Review and audit health coach data collection and clinical notes. 12.Expand program to Nuvance West (hiring and training new BHC staff, update workflows, attend and facilitate meetings with IT and clinical teams for program success). 13.Complete administrative forms for schedule changes and credentialing for new employees; IT issue/service/equipment requests. 14.Reinstate MAT program and workflows to continue spreading MAT across network 15.Pursue grant funding to support SBIRT and MAT programs. 16.Hire, train and supervise a health coach and opiate navigator for Nuvance West. 17.Support and provide open communication to specialty providers to improve access to Mental Health Services via the BHC staff in Primary Care. 18.Train, supervise and develop new BHC role that will support the metabolic program. 19.Train, hire and develop new BHC who will support endocrinology in their pursuit to become a Center of excellence for Diabetes management. Establish annual goals for staff. 20.Create and develop workflows for residency program to interface with PC providers through the BHC staff and provide monitoring to promote optimization and program success. 21.Attend and participate in SBIRT steering committee meetings monthly for continued review of SBIRT processes for improved optimization for patient care both for Nuvance East and West. 22.Participate in all meetings related to PC and BH (Primary care leadership, provider or staff meetings in Nuvance PC offices for BH program review and SBIRT trainings, grand rounds, specialty offices meeting, Schwartz Rounds, MAT and High Risk Opiate Task force meetings Nuvance West). 23.Maintain all CEUs for both CT and NY license requirements and maintain Grief Counseling Certification yearly.
