Transfer Center Manager
Baystate Health, Western Massachusetts’s premier healthcare provider, and home to Baystate Medical Center (BMC), a Magnet Hospital for Nursing Excellence seeks a Full Time Transfer Center Manager
The Transfer Center Manager at Baystate Health collaborates closely with medical staff, administrators, department heads, nursing teams, and external facilities to facilitate timely patient transfers and placements within the health system.
This crucial role involves leading, organizing, and motivating the transfer and placement team. Utilizing health data management skills and workflow analysis, the Manager drives continuous process improvements to optimize patient flow into and within Baystate, ensuring efficient operations and high-quality care delivery.
Requirements
• Build and maintain a high-functioning Patient Placement/Transfer Center team, ensuring all members work together to assure all aspects of patient placement occur in an organized and timely manner.
• Manage the day-to-day operations of the Patient Placement/Transfer Center including the Patient Placement Technicians and Patient Placement (RNs), with accountability and responsibility for new hires, performance evaluations, and verification of competency in their assigned duties.
• Demonstrate expert knowledge in patient placement, varying levels of care required depending on patient's status and clinical needs, and service availability at Baystate Health facilities.
• Oversee and Manager, ensuring timely and safe patient transfer from referring institutions.
• Identify trends or changes that may affect staffing resources and assist in developing strategies to address changes in demand.
• Use TeleTracking and other informatics tools to monitor patient flow and bed management, ensuring adherence to current policies and compliance standards.
• Design and document workflow requirements, process flows, and system testing to ensure operational excellence.
• Act as a strategic liaison between Patient Placement and various internal and external entities within Baystate Health, fostering collaboration among clinical and business partners.
• Collaborate with clinical and business partners to develop sound operational solutions and propose application changes/enhancements to meet team/service line demands.
• Analyze clinical operations and processes to facilitate decision-making regarding, workflows, clinical systems integration, ensuring systems are consistent with professional practice standards.
• Develop and implement workflows, system analysis, and other informatics solutions to improve patient flow, quality of care and overall team efficiency and effectiveness.
• Provide leadership for complex projects, ensuring adherence to established plans and evaluating project success through post-implementation assessments.
• Ensure quality, evidence-based care is provided, and patients are assigned to the appropriate level of care with the correct status.
• Participate in Quality Management initiatives, assist in data collection and support metrics for the patient throughput department, and uphold high standards of performance and accountability.
• Ensure Baystate Health is evaluating and selecting key emerging technologies that promote improved healthcare delivery models coordinated with effective and efficient workflows.
QUALIFICATIONS:
Bachelor’s degree in nursing and RN MA license Minimum of 5 years of acute care nursing Leadership experience and/or prior experience in a transfer center is strongly preferred. Candidates with a proven ability to facilitate throughout and guide teams in a fast-paced environment will be highly valued Excellent interpersonal skills for interaction with all levels of healthcare providers and staff Ability to organize and prioritize workload, meet deadlines, and work in a fast-paced environment. Strong critical thinking skills with the ability to assess and prioritize decisions rapidly. Proficient in computer systems, databases, and telephone communication. Demonstrated ability to foster a team environment and lead successful change initiatives. Technical and professional knowledge, customer focus, communication, building partnerships, and critical thinking. Ability to assess, map, and analyze clinical and business practices to informatics applications.SCHEDULE:
