Manager, Rehabilitation Therapy
**Employment Type:** Full time **Shift:** Day Shift **Description:** **Manager, Rehabilitation Therapy** **Location:** St Francis Healthcare, Wilmington, DE **Status:** Full time **Shift:** Mon to Fri, 8 am to 430 pm, no nights/weekends/on call **Position Purpose:** The primary purpose of this position is responsibility for providing strategic direction and oversight to the therapy services offered in the program. Assures therapy services function as an integral part of the interdisciplinary team (IDT) and models professional behavior related to collaboration and coordination of services. **Position Details:** Home visits are required at times based on the participant’s care plan and need for rehab assessment in the community. Percentage of home care visits are determined based on participant need. Travel required throughout New Castle County, DE. Other local travel may be required. **What you will do:** + Oversee daily therapy department operations including staffing, scheduling, assessments, and referrals. + Participate in IDT assessments (initial, semi-annual, unscheduled, annual) to ensure rehab needs are identified and addressed through effective care planning and implementation. + Develop and maintain care plans that integrate therapy services and meet regulatory standards. + Provide clinical guidance on therapy-related aspects of participant care. + Participate in Quality Assessment and Process Improvement (QAPI) initiatives. + Ensure compliance with federal and state regulations for PACE and adult day care services. + Maintain accurate and timely documentation of participant care. + Manage rehab services efficiently, including authorizations and collaboration with finance. + Support survey readiness and contribute to corrective action plans when needed. + Ensure safety protocols are followed during care delivery. + Assess and coordinate durable medical equipment and specialized footwear needs. **Minimum Qualifications:** + Active, unencumbered license in the state of the PACE organization, + Bachelor's in Physical or Occupational Therapy. + Experience: 2+ years as a PT/OT. + Supervisory experience preferred. + 1+ year with frail/elderly populations (or training provided); homecare experience preferred. + Strong decision-making, adaptability, conflict management, and customer focus. + Effective in cross-functional teams and virtual environments; strong interpersonal and leadership skills. + Excellent written and verbal communication tailored to diverse audiences. + Skilled in Microsoft Office; familiarity with electronic documentation systems. + High personal responsibility and independent judgment; capable of strategic planning and implementation. + Strong multitasking, problem-solving, and prioritization skills in dynamic settings. + Able to research, analyze, and apply information effectively. **Position Highlights and Benefits:** + Comprehensive benefits including 1st Day medical coverage, dental, vision, paid time off, 403B and educational assistance. + Access to daily pay and employee referral incentives. + Supportive environment with a patient-centered focus. + Opportunities for professional development. **Ministry/Facility Information** **Saint Francis LIFE Riverfront** , part of Trinity Health PACE, provides high-quality care to seniors in the communities we serve.
