Position:Manager, Population HealthPosition Summary:In this impactful role, you will champion Accountable Care Team (ACT) engagement by providing leadership, guidance, and support to help physicians, nurses, administrative staff, and management collaborate effectively.You’ll lead a dedicated team focused on increasing awareness, adoption, and utilization of cutting-edge provider engagement technology, quality reporting tools, and performance optimization solutions. Your efforts will foster a culture of continuous development and shared "mindshare" with ACT partners helping them deliver exceptional, value-based care.
As a key influencer, you’ll inspire and enable ACT teams to achieve meaningful improvements in quality measurement, efficiency, and cost stewardship. You will also play a vital role in shaping strong, collaborative relationships with our physicians at the practice level making a real difference in the future of healthcare.Job Description:
Primary Responsibilities
Manages the activities of and coaches, mentors, motivates, supports, and develops a team of individual contributors and/or supervisors. Manages and/or supports performance management/corrective action plans, goal setting/review, regular guidance and 1-1 check ins, career development/planning, upskilling, resource allocation, staffing, DE&I initiatives, and other People & Culture programs/activities as needed. Fosters accountability and collaboration to ensure team member and overall team success.Establishes strong personal relationships with external Accountable Care Teams and internal subject matter experts.Collaborates to develop and implement strategic initiatives to improve performance against value-based contract metrics.Performs ongoing program management and evaluation for program effectiveness.Supports JOC meetings.Facilitates best practice sharing across teams.Supports sustainable clinical practice transformation including practice observations, review of findings, best practice recommendations and provision of tools and resources to promote population health management and value-based care.Responsible for customer defining adoption baseline and yielding improvement.Develops and delivers programs, training, and adoption to client prospects.Supports sales with subject matter expertise, content and materials related to client and implementation services.Provides expertise and guides change management.Leverages relationships with industry partners and thought leaders to support sales of technology and consulting solutions.Responsible for deliverables of client engagements, including project timelines.Provides subject-matter expertise in program development and solution packaging.Owns and develops client relationships at the C-Suite level.Keeps abreast of current industry, market, and policy landscape.Qualifications
Bachelor's degree in relevant field (i.e., Healthcare or Business) or equivalent6+ years of client facing experience within the healthcare domain or the knowledge, skills, and abilities to succeed in the role1+ years of leadership experience or the knowledge, skills, and abilities to succeed in the roleSkilled at working with physician practices including; provider workflows, provider relations, provider education, practice transformation and/or practice managementAdvanced knowledge of the U.S. healthcare systemSubject matter expertise in any of the following areas: Medicare Advantage, Medicare Shared Savings Program (MSSP), managed care regulations, quality reporting, care management, population management, ACO strategy implementation and operations, supporting providers in risk-based contracts, and patient centered medical home (PCMH)Strategic and entrepreneurial outlook with strong collaborations skillsAbility to guide client interactions at C-levelAbility to operate in a fast-paced and dynamic environmentTeam player, excellent communication skillsStrong client-facing skillsStrong presentation skillsPreferred

In addition to the base salary, certain roles may qualify for a performance-based incentive and/or equity, with eligibility depending on the position. These rewards are based on a combination of company performance and individual achievements.
The hiring range for this position is:
