Job Overview:

We are seeking an experienced Manager to oversee a team of Community Health Workers who will be responsible for completing Social Determinants of Health screenings with patients in the hospital setting. The primary objective of this position is to improve access to healthcare services and reduce hospital readmission. This is a grant-funded position until 2025 year-end, strong possibility of future extensions.

Qualifications:

Required:

Bachelor's degree in healthcare administration, public health, or related field

Minimum of 3-5 years of experience in public health or healthcare administration

Strong leadership and management skills with the ability to motivate and inspire a team

Excellent communication and interpersonal skills

Ability to analyze data and make data-driven decisions

Familiarity with healthcare regulations and patient privacy laws

Knowledge of medical terminology and healthcare procedures

Ability to work collaboratively with clinical staff and other stakeholders

Proficiency in Microsoft Office Suite and electronic medical records systems

Scheduling Requirements: Primary physical working location is Community Medical Center (CMC) in Toms River, NJ. This role requires local travel within the Southern Region to include coverage/support at MMC, MMCSC, & HAM as needed.

Essential Functions:

Manage and lead a team of Community Health Workers to meet program goalsTravel between multiple hospitals and organizations in the region regularlyDevelop and implement process improvement plans to increase patient engagement and complianceAnalyze and report on program effectiveness and recommend adjustments as neededCoordinate team member schedules across multiple facility locationsCollaborate with clinical staff to ensure that patients receive timely and appropriate careRegularly meet with team members to review performance and metricsProvide regular supervision sessions with CHWsTrack and report on patient outcomes and the effectiveness of the program to senior leadershipEnsure that all Community Health Worker activities comply with healthcare regulations and patient privacy lawsMaintain accurate records of all Community Health Worker and patient interactionsFoster strong relationships with key stakeholders by participating in regular meetings to enhance quality services and promptly address requestsConnect with community organizations to maintain and facilitate strong relationships
