The primary objective of this position is to improve access to healthcare services and reduce hospital readmission. This is a grant-funded position until 2025 year-end, strong possibility of future extensions. Qualifications: Required: + Bachelor's degree in healthcare administration, public health, or related field + Minimum of 3-5 years of experience in public health or healthcare administration + Strong leadership and management skills with the ability to motivate and inspire a team + Excellent communication and interpersonal skills + Ability to analyze data and make data-driven decisions + Familiarity with healthcare regulations and patient privacy laws + Knowledge of medical terminology and healthcare procedures + Ability to work collaboratively with clinical staff and other stakeholders + Proficiency in Microsoft Office Suite and electronic medical records systems Scheduling Requirements:This role requires local travel/support between New Brunswick and Somerset as needed. Essential Functions: + Manage and lead a team of Community Health Workers to meet program goals + Travel between multiple hospitals and organizations in the region regularly + Develop and implement process improvement plans to increase patient engagement and compliance + Analyze and report on program effectiveness and recommend adjustments as needed + Coordinate team member schedules across multiple facility locations + Collaborate with clinical staff to ensure that patients receive timely and appropriate care + Regularly meet with team members to review performance and metrics + Provide regular supervision sessions with CHWs + Track and report on patient outcomes and the effectiveness of the program to senior leadership + Ensure that all Community Health Worker activities comply with healthcare regulations and patient privacy laws + Maintain accurate records of all Community Health Worker and patient interactions + Foster strong relationships with key stakeholders by participating in regular meetings to enhance quality services and promptly address requests + Connect with community organizations to maintain and facilitate strong relationships Other Duties: Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
