Responsibilities

The incumbent provides care, treatment and service to Home Health and Hospice patients and their families in their place of residence within thier scope of practice. Functions as a member of the interdisciplinary team.

Qualifications

Experience: 

2 years LVN Experience within the last 5 yearsRecent experience as an LVN in a Home Health or Hospice Agency- preferred.

Licensure:

Currently licensed as a LVN in CaliforniaCurrent California driver's license, possession of automobile insurance, & access to an operating motor vehicle.Current CPR/BLS card approved through the American Heart Association.Overview
