Qualified applicants must live in one of these states: Iowa, Illinois, Indiana, Kansas, Michigan, Minnesota, Missouri, Nebraska, South Dakota or Wisconsin.
The Clinical Intake Coordinator is responsible for timely and accurate data entry of all hospice referrals into the electronic software system; tracking all pending referrals until admission or non-admission; obtaining medical records; sending electronic consents and instructing POA in completion of consents. The Intake Clinical Coordinator will play a vital role in reviewing clinical records to ensure the RN scheduled to assess the patient's eligibility is set up for success with proper documentation and clinical records to support the admitting hospice diagnosis.
Essential Functions
1. Receives patient referrals via fax, phone and email and enters into HCHB in a timely manner.
2. Establishes and maintains a positive working relationship with current and potential referral sources.
3. Facilitates and schedules the nurse evaluation by coordinating with patients, families, facilities, and branch staff.
4. Obtains medical records via fax requests and/or via other health organizations EMR system.
5. Reviews referrals for clinical needs to assist and prepare the RN for the evaluation.
6. Coordinates hospital discharge ensuring a smooth transition to patient's home.
7. Orders DME and other supplies when appropriate.
8. Assists with referral management including scheduling evaluations to ensure timeliness to the patient's bedside.
9. Addresses and resolves any barriers to admission.
10. Maintains all updates for pending referrals.
11. Reviews DocuSign paperwork with patient's POA if they are unable to be at the evaluation.
12. Accountable for compliance with all HIPAA, CMS, CHAP and CAHPS regulations and standards.
13. Performs other related duties as assigned.
