LPN/LVN - Hospice After Hours (LA County)

This position will be traveling after hours to see patients in the Los Angeles county area.

The Licensed Practical/Vocational Nurse is responsible for providing direct patient/family care under the supervision of a Registered Nurse for the Hospice Care Program. Responsibilities include following the Plan of Care, providing treatments, and working collaboratively with the members of the team to help meet positive patient/family care outcomes. May provide care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.

Required qualifications:

Graduate of a Vocational Nursing program.  Upon hire: California Licensed Vocational Nurse  Driving is a necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.Within 6 months (180 days) of hire:  Phlebotomy/IV Certification required. Must obtain within 6 months of hire.1 year of Nursing experience.

Preferred qualifications:

Community health/hospice/palliative care or medical/surgical experience.
