We are seeking a LPN- Licensed Practical Nurse to support our residents in our Skilled Nursing Facility (Transitional Care) in Burnsville, MN! This role performs a variety of professional nursing duties in compliance with the Comprehensive Home Care regulations and follows established policies and procedures. Responsibilities: Provides skilled nursing care such as; implementing practitioners’ orders, ordering and assisting with medications and supplies, operates medical equipment, and charting practices Lifts and transfer residents as needed, Initiates emergency treatment as needed Monitors work and performance of unlicensed staff Updates RN with current observation and completes documentation of resident health status Communicates changes in residents conditions to relevant individuals May support RN by completing resident assessments, service agreements and care plans Required: MN LPN licensure AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers Preferred: Able to work independently Good communication and interpersonal skills Previous healthcare experience Comfortable using computers, hand held devices, phones and fax machine Join our dedicated team and contribute to our mission of helping seniors heal, discover, and lead longer, healthier, and more meaningful lives.
